Accomplish is a specialist, outcome focused provider of support for people with Mental Health needs, Learning Disabilities, Autism and Acquired Brain Injuries. We work in partnership with commissioners to deliver responsive, flexible, specialist support in registered residential and supported living settings across England and Wales. Our individual support plans aim to provide the people we support with more choice and control over their own lives.

Our individual support plans aim to provide the people we support with Learning Disabilities more choice and control over their own lives. Encouraging individuals to have a belief in reaching their full potential regardless of their ability within a structured, holistic environment.

We have several services across England and Wales with a wealth of experience and expertise in supporting people who have a dual diagnosis of mental health or challenging needs associated with their learning disability.

Clinical Support

Our services undertake an in-depth assessment and analysis of challenging needs using the B-BAT tool and take a positive approach based on detailed behaviour support planning. Each person is supported with an individual care plan designed to meet their individual needs. Our learning disability services benefit from the support of Tracscare’s Behaviour Advisors who work intensively alongside a team to support that team and the individual for extended periods if required.

They review the person's positive behaviour support plan in response to any behavioural change. They work alongside the staff team but also in conjunction with the wider multidisciplinary care team. The role of the Behaviour Advisor is significant in maintaining placements when challenges arise and in supporting the individual to engage in positive behaviours and to meet their needs. This is achieved through functional behaviour analysis or engaging effective communication strategies.

We provide person-centred residential support to people with highly challenging learning disabilities in homely environments in different styled settings within the heart of their local communities. Tracscare is committed to a philosophy of ‘doing with’ rather than ‘doing for’ and promotes the active support model.

Active Support Model

This service model encourages the person involvement in every aspect of their life and through planned interventions and mentoring enables skill development. Active support is outcomes focussed and allows for monitoring of progress towards maximised participation. Our person-centred approach has led to individuals fully engaging in their review process and making positive life changes based on their person-centred profiles.
